Registered Nurse — St. Patrick Center ACT Program

St. Patrick Center, a ministry of Catholic Charities is hiring a full-time Registered Nurse. This RN will provide professional, quality program services to the clients served by the Assertive Community Treatment (ACT) program. In this role you will promote the physical and mental health of clients on the ACT Team Two and work with other members of the treatment team to assist clients to obtain and maintain an optimum quality of life. This position will provide clinical oversight to staff by monitoring, educating, coaching, and instructing clients as needed. The core working hours are Monday-Friday, 8:00 AM-4:30 PM.

Overview and Responsibilities

  • Provide consultation for ACT team regarding psychiatric symptoms, co-occurring medical conditions and appropriate interventions
  • Conduct comprehensive, ongoing assessment of clients’ physical health and medical history
  • Manage primary case load by completing treatment plans, DLA-20’s and crisis plans
  • Provide direct clinical services to clients individually in office and community settings
  • Refer clients for appropriate specialists or primary care physicians
  • Collaborate with ACT team in daily staff meetings and treatment planning
  • Take lead role in medication administration and medical services
  • Develop, maintain and supervise psychopharmacologic treatment policies and procedures
  • Administer IM medications and complete lab work as ordered
  • Manage pharmaceuticals and medical supplies
  • Collaborate with other health providers for continuity of care
  • Work with ACT psychiatrist to coordinate care
  • Provide on-call crisis intervention covering nighttime, weekend, and holiday hours
  • Maintain client tracking system and files, document notes within 24 hours
  • Other duties as assigned

Knowledge and Experience Requirements

  • Licensed Registered Nurse required
  • Drug test within 30 days of hire
  • Requires driver license (MO residents require Class E)
  • Requires valid auto insurance and reliable vehicle

Skills Required

  • Understanding of mental health and substance use disorders
  • Experience working with the unhoused community
  • Knowledge of federal, state and agency policies of client confidentiality
  • Excellent communication skills
